Due to the sensitive nature of the data, all requests have to be made in person and the relevant original documents are required:

- For change of name, please provide original photo ID issued by govrement.
- For changes in residential address, please provide one of the following documents (within the last 3 months)
    1 phone bill/Internet bill/ Utilities bill
     2 Statement/letter from your Bank
     3 Credit Card / Insurance Statement
     4 Tenancy agreement for residence
